Taking multiple medications? Recently discharged from hospital? Have questions about your medications? Leumit’s pharmacists are here to help!
Leumit offers personal consultations with a pharmacist, providing guidance and support with your medication treatment. The consultation aims to help identify medication-related problems, improve your understanding of your treatment, support safe and correct medication use, and answer your questions about taking your medications. The service is provided by appropriately trained pharmacists, according to your medical needs.
Who can benefit from a pharmacist consultation?
The service is for patients who would like information and guidance about their medications, particularly those who:
- Regularly take more than six medications
- Have recently been discharged from hospital or had a significant change in their medication treatment
- Would like explanations and guidance about the medications they take
What does the consultation include?
During the consultation, the pharmacist reviews your medical record to identify potential medication-related problems and, where necessary, contacts your doctor to share recommendations.
The entire process is carried out in full collaboration with your treating doctor.
You can also receive information and guidance on:
- How to take your medications correctly, including when and in what order to take them
- Possible side effects
- Interactions between medications, dietary supplements and food
- Understanding your treatment and following your medication regimen
At the end of the consultation, you will receive an up-to-date medication chart.
The service gives you an opportunity to have your questions answered and helps improve the quality of your care.
How much does a pharmacist consultation cost?
The service is free of charge.
Where are pharmacist consultations available?
Leumit members can receive consultations:
- Remotely: By phone or video call (Zoom)
- In person: At pharmacies in selected Leumit branches
How can I request a pharmacist consultation?
You can request a consultation in either of the following ways:
Contact your Leumit pharmacy
Contact your designated Leumit pharmacy and ask for a pharmacist consultation. If in-person consultations are not available there, you will be offered a consultation at another pharmacy or a remote consultation.
Complete the online form
Fill out the short request form at the bottom of this page. Once we receive your request, we will contact you within seven working days to confirm the details and arrange your consultation.
What else should I know?
The service is not intended for urgent situations and does not replace medical care.
If you have an urgent medical problem, your health worsens, or you suspect an unusual reaction to a medication, contact your treating doctor, an urgent care service or an emergency department, as appropriate.
Consultations are subject to professional assessment and service availability.
