Referral by a Doctor
In cases where the treating physician or another healthcare professional identifies a mental health issue that requires evaluation, they can refer patients to an adult or child psychiatrist.
Book an Appointment Directly
If you have not received treatment in the past six months, please schedule an appointment with a mental health support coordinator by calling *507.
The mental health support coordinator will assist with the assessment process and with matching the appropriate treatment according to your preferences and availability in your area of residence.
Requesting a Referral Form
A referral form can be requested through the website, app, or medical center.
For continued treatment with a psychiatrist within the Leumit network via direct appointment booking, no referral form is required.
For continued treatment by a psychiatrist at an external institute or clinic, a referral form (membership or commitment) is required.
For psychotherapy treatment at Leumit medical centers, external institutes, clinics, or providers, a referral form is required.
