A member who requires a healthcare service that is not included in the Government Health Basket, as defined by law, may submit a request to the Exceptions Committee.
The Exceptions Committee reviews requests from members who have a medical condition that has a substantial impact on their health status and daily functioning, where the requested service is expected to result in a significant improvement in their condition.
In order for the Committee members to review the request, an Application for Review by the Exceptions Committee must be completed.
Please ensure that the form is completed fully and in detail, to enable the Committee to reach a professional and well-founded decision.
