Transition from Hospitalization to Community-Based Rehabilitation
After hospitalization, there are several options for rehabilitation within the community.
What Is Rehabilitation?
Rehabilitation is a therapeutic process designed to help clients improve motor, cognitive, and emotional functions that were affected by illness or injury, and to support their optimal return to daily life. Rehabilitation takes place after discharge from the hospital, with the goal of restoring the highest possible level of independence.
After discharge, and based on the recommendation of the professional medical team, a joint decision with the Leumit team will determine the appropriate rehabilitation pathway.
There Are Four Main Rehabilitation Options
Institutional Rehabilitation (Inpatient Rehabilitation)
- Intended for clients who require intensive, multidisciplinary care
- Provided in dedicated rehabilitation centers within hospitals or specialized institutions.
- Includes a multidisciplinary team: rehabilitation physicians, physiotherapists, occupational therapists, speech and language therapists, psychologists, and dietitians.
- Suitable for conditions such as brain injuries, stroke, complex orthopedic injuries, and advanced neurological diseases.
Outpatient Rehabilitation
Designed for clients who need intensive, multidisciplinary rehabilitation without 24/7 medical supervision.
- Provided in hospitals or dedicated day-rehabilitation centers.
- Includes at least three health professions: physiotherapy, hydrotherapy, occupational therapy, speech therapy, nutrition, and/or emotional therapy.
- Suitable for clients in stable medical condition who can manage several consecutive intensive sessions each visit.
- Clients must arrive independently and stay at the rehabilitation center for several hours, several times per week.
Home-Based Rehabilitation in the Community
For clients who do not need an institutional setting but do require rehabilitation.
- Suitable for situations such as post-operative recovery, mild stroke, orthopedic injuries, functional decline after hospitalization, and more.
- Provided as part of rehabilitation packages by authorized providers.
- Includes multidisciplinary care at the client’s home by rehabilitation physicians, nurses, physiotherapists, occupational therapists, speech therapists, dietitians, social workers, according to medical need.
Individual Treatments Provided at Home or in the Community
For clients who require focused treatments only.
- Provided at Leumit clinics or at the client’s home.
- Includes services such as physiotherapy, occupational therapy, speech therapy, and nutrition counseling.
- Suitable for mild injuries, gradual rehabilitation following institutional treatment, or recovery after elective surgeries such as hip or knee replacement.
Benefits of Home-Based or Individual Community Rehabilitation
Home-based rehabilitation and individual community treatments offer several significant advantages:
- Clients remain in a familiar, natural environment.
- Treatments are integrated into the personal routine of clients and their families, with flexibility according to their needs.
- Family members and primary caregivers can take part in the rehabilitation process, enabling a more comprehensive and effective program.
- No need for travel or long stays in treatment facilities.
How to Begin: Requesting Rehabilitation
A rehabilitation recommendation is provided by the hospital’s medical team together with the Leumit team.
- Institutional rehabilitation: Leumit will coordinate the client’s transfer to the appropriate rehabilitation facility.
- Outpatient rehabilitation: The client receives a referral from the hospital department to their personal physician. The physician submits an approval request to the Approvals Center. The client then coordinates treatment directly with the day-rehabilitation center.
- Home-based rehabilitation: After arriving home, the provider will contact the client to schedule the first visit. Initial treatments are provided within 24 hours of returning home.
